Distinguished Lecture Series 2021 - Reforming the Health Systems in States in the Background of the Pandemic documents the inaugural lecture of the Public Affairs Centre's Distinguished Lecture Series, delivered by Dr. Sujatha Rao, IAS (Retd.), former Secretary, Ministry of Health and Family Welfare, Government of India. The lecture examined the lessons emerging from the COVID-19 pandemic and their implications for strengthening India's state health systems. It highlighted the importance of reliable and transparent health data, evidence-based decision-making, decentralised governance, public health expertise, and effective inter-sectoral coordination in managing public health emergencies. Drawing on the experiences of states such as Kerala, Karnataka, and Tamil Nadu, the lecture discussed innovations in pandemic management, including technology-enabled surveillance, telemedicine, laboratory expansion, district-level planning, and community participation, while identifying persistent challenges such as inadequate public health expenditure, weak district health infrastructure, shortages of trained human resources, interstate inequalities, and disruptions to non-COVID healthcare services. The report concludes by emphasising the need to redesign India's health system around a preventive, decentralised, and public health-oriented approach that prioritises resilience, equitable access, stronger primary healthcare, improved data systems, and sustained investment in public health institutions to better prepare for future health emergencies.
